Athletic Participation Requirements
Student athletes who are interested in trying out or joining a team will need to have all required paperwork turned in by the first day of practice/try outs. Turn paperwork & money into your coach or the Athletic Office.
ALL REQUIREMENTS MUST BE MET:
1 - All sport participants must have a Participation Form filled out and on file in the athletic office before they can try out for a team.
2 - Updated physical information (page 2 of the participation form above). Physical must be after April 1, 2012.
3 - A $100.00 (non-refundable) participation fee is due for each sport, with a family cap of $400. Checks can be made out to MHS. Please note: If your sport requires you to try out, this fee is due the first day of practice after try outs.
4 - Read and sign the Concussion Form for each sport an athlete goes out for. Reference the Parent Concussion and Athlete Concussion information flyers.
